PHYSICIAN COVERAGE
Dr. King offers vascular care and evaluation around the clock. A qualified vascular surgeon is available 24 hours a day, 7 days per week. All emergencies, from ruptured aneurysms to life threatening infections to vascular trauma, are covered. During hospitalization, attending physicians supervise the house staff, who are residents in general and vascular surgery at various levels of postgraduate training. For the many kinds of vascular conditions not requiring surgical intervention, innovative techniques using specially designed balloons and catheters can often restore circulation. Because vascular surgeons are trained in all forms of treatment, from medical regimens to open surgery to catheter guided procedures, Dr. King is qualified to offer patients a variety of therapeutic options best suited to their individual cases. He shares a call schedule with other vascular surgeons who maintain privileges at the hospitals.

Terry A. King, MD, F.A.C.S., RVT. Dr. King is board certified in both vascular and general surgery and is also a registered vascular technologist. He is an Associate Professor of Surgery at Case Western Reserve University, past president of the Cleveland Vascular Society and author of several clinical surgery textbook chapters and articles. Following a five-year General Surgery Residency at Case Western Reserve University in Cleveland, Dr. King received an additional year of specialty training as a Fellow in Vascular Surgery at Northwestern University Medical Center in Chicago, Illinois. He worked at the George Washington University Hospital and Veteran's Administration Hospital, both in Washington, DC before returning to Cleveland. He has taken additional course work in endovascular interventional techniques.

Dr. King practices in all areas of peripheral vascular surgery, but his special areas of interest are endovascular intervention, venous disease and arterial occlusive disease.

APPOINTMENTS (216) 839-3800
Patients are seen by appointment at the Beachwood office located at the Cleveland Clinic Fooundation Family Health Center at 26900 Cedar Road, Suite 32-N, Beachwood Ohio, 44122. near the intersection of Cedar Road and I-271, next door to the Beachwood Place Mall. You may call the office Monday-Friday from 8:30 a.m. to 4:30 p.m. to schedule an appointment. Regular office visits are scheduled for Monday afternoons, Wednesday mornings, and all day Friday. Try to call at least two weeks in advance of when you would like to be seen. This should enable the staff to schedule an appointment at a time convenient for you. Of course, emergencies are always given priority and worked into the daily schedule. On these more urgent occasions, Dr. King may see you at a hospital examination room between surgeries or have an associate evaluate you if he is unable to do so himself.

If you are unable to keep you appointment, please notify us at least 24 hours in advance so that other patients can be scheduled. We value your time and will make every effort to see you as scheduled. However, complex cases may occasionally cause delays. On these rare occasions, the receptionist will attempt to contact you if there will be a significant wait or rescheduling is necessary.

Should your care require hospitalization, we will arrange for admission. Outpatient surgical procedures are performed in the Ambulatory Surgery Center of the Beachwood Family Health Center or one of the Cleveland Clinic affiliated hospital operating rooms. Injection sclerotherapy is performed in the office setting. Vascular laboratory appointments are made by calling directly to (216) 839-3800, but can only be scheduled on the advice of a physician.

TELEPHONE CALLS TO THE DOCTOR & NURSES
You are encouraged to call Dr. King or his nursing staff with any questions. All calls and prescription refill requests should be made during office hours whenever possible. Physician callbacks will usually be made after office hours and often nursing callbacks are used first to determine the precise nature of the request when the doctor is in surgery. When calling, please leave your telephone number, including area code, and what time is best to reach you in the evening.

EMERGENCIES
If you are injured or urgently ill and must be seen immediately by a physician, please go directly to the Emergency Department of your closest hospital. The Emergency Department is staffed by specialists and equipped to handle your care. Their staff will locate Dr. King or the vascular surgeon on call to inform him of your condition.

In case of a non life-threatening emergency during business hours (Monday-Friday, 8:30 a.m. to 4:30 p.m.) please enlist the aid of the office staff to locate Dr. King. If you have a medical emergency after hours or on the weekend, the on-call vascular surgeon can be reached through the answering service operator at (216) 839-3800.

PATIENT REGISTRATION
New patients will be asked to fill out a Patient Registration Form. A portion of this information is confidential and will be released only upon your written authorization.

You will be asked to complete a health questionnaire related to your general medical history. These questions were designed to give the doctor and nurse important information regarding the background of your problem and any previous treatment. Completion of this questionnaire gives you a chance to organize your thoughts and to be sure to remember to tell the doctor about the symptoms, history and treatment of this complaint, past illnesses and other pertinent medical history. It is very helpful to bring a list of current medications and allergies with you. For your convenience, a Personal Health History Form is located in the General Information Section.

FEES & INSURANCE
Dr. King too is concerned about the cost of quality medical care and he does everything possible to keep fees reasonable. Billing and payment policies are summarized below. Surgical procedures are billed to your insurance company. He participates in Medicare and will file Medicare claims. The practice participates in many plans, so be sure to ask the receptionist about your coverage. If your insurance requires a referral from your primary care physician, please bring it with you to the office. Lack of an appropriate referral can cause your appointment to be canceled and rescheduled in non-urgent situations.

PAYMENTS
Co-payments are expected for all routine visits at the time of service. This reduces the administrative expenses and helps to contain medical costs. While surgical procedures will be billed to your insurance company, it is your responsibility to pay any deductible fees, co-payments, or any balance not paid by your insurance company.

Cosmetic procedures such as injection sclerotherapy are not covered by insurance. Some procedures, however, may qualify as tax-deductible medical expenses. Your receipt includes all the information needed to document the procedure for your taxesrecords.

For your convenience, the office accepts personal checks, cash, Visa, and MasterCard.
